3. I agree to reschedule if I have experienced any cold or flu-like symptons within 14 days of the appointment.*

4. I agree to help prevent the spread of this virus by following salon guidelines and wearing a mask at all times.*

5. I understand there will be a $5 COVID-19 surcharge to offset the cost of PPE and CDC disinfecting protocols.*
